29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) Lakeview Forge Company, LLC - On or about August 16, 2017, employer had not obtained annual audiograms for employees exposed to noise above the action level. On or about September 27, 2017, the following noise exposures were measured: 1. A hammerman was exposed to a mixed noise level of 105.5 dBA or an equivalent dose of 859.8%, as an 8-hour TWA, while conducting forging operations. This level was based on 435 minutes of sampling; zero exposure was considered for the unsampled time. 2. A trimmer was exposed to a mixed noise level of 102.8 dBA or an equivalent dose of 592%, as an 8-hour TWA, while conducting forging operations. This level was based on 439 minutes of sampling; zero exposure was considered for the unsampled time. 3. A hammerman was exposed to a mixed noise level of 97.3 dBA or an equivalent dose of 275%, as an 8-hour TWA, while conducting forging operation. This level was based on 433 minutes of sampling; zero exposure was considered for the unsampled time. 4. A fork-truck operatior was exposed to a mixed noise level of 94.6 dBA or an equivalent dose of 190.3%, as an 8-hour TWA, while conducting fork-truck operations. This level was based on 427 minutes of sampling; zero exposure was considered for the unsampled time.

The employer did not institute a training program and ensure employee participation in the program: a) Lakeview Forge Company, LLC - the employer did not provide hearing conservation training for employees exposed to noise above the action level.
