Lakeview Forge Co., LLC dba Erie Tool Works; drill press area - On or about 1/23/19, and at times prior thereto, a 230 volt electrical disconnect was missing two knockouts. The box provides power to electrical outlets in the maintenance department.

Lakeview Forge Co., LLC dba Erie Tool Works; heat treat area - On or about 1/23/19, and at times prior thereto, the area in front of 230 volt electrical disconnects was being used for storage . Items being stored in front of the disconnects included, but were not limited to, a uniformity tree, a cart, and card board boxes.
