29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  325 Whippany Road, Building 3, Whippany Crossing, Whippany, NJ.  Employees were exposed to falls of 12'5" from the top of the ladder to the ground below as the ladder side rails did not extend at  least 3 feet above the first floor roof's surface.                                                 Violation observed on or about 02/27/2017.

Employees were exposed to falls of 12'5" from the top of the ladder to the ground below as the ladder was used inverted with its foot end at the roof level which is not the intended use of this ladder.                                                 Violation observed on or about 02/27/2017.

29 CFR 1926.1053(b)(16):   Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a)  325 Whippany Road, Building 3, Whippany Crossing, Whippany, NJ.  Employees were exposed to falls of 12'5" from the top of the ladder to the ground below as the ladder was missing a section of its rail along with one of the ladder feet.                                                 Violation observed on or about 02/27/2017.  b)  325 Whippany Road, Building 3, Whippany Crossing, Whippany, NJ.  Employees were exposed to falls of 12' from the top of the ladder to the ground below as the ladder was missing a rung which could lead to structural failure of the ladder.
