29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 09/01/17, on the right side of a house at 4 Woodgate Drive, Lancaster, NY: Two separate ladder jack scaffolds were erected on the right side of the house, end to end. The scaffold closest to the rear of the house had a guardrail on the outside of the aluminum pik, but had no guardrail on each end of the pik. The scaffold closest to the front of the house had no guardrails on its pik. No personal fall arrest system or other fall protection system was in use on either scaffold. Employees were exposed to a fall of approx. 15 ft. to the ground. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 09/01/17, on the roof on the right side of a house at 4 Woodgate Drive, Lancaster, NY: Four employees engaged in roofing work on a steep roof were exposed to a fall of approx. 18 ft. Three of the four employees were wearing full body harnesses , but had no lanyards attached. None of the employees were tethered to an anchor point. Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED
