29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about December 20, 2017, employee was exposed to ingoing nip-point hazards during the repair of the MG-01 Dust Collector. The employer failed to implement all energy control application steps when kinetic energy was not dissipated prior to performing the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, dissipation of residual energy [(d)(5)(i)], was not implemented to protect employees from machine servicing hazards.
