29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:  (a) HAC Roofing, LLC at 5025 Lowell Blvd. Denver, CO 80221: On or before September 19, 2017 the exposing employer did not protect employees from injury, in that employees removing and installing shingles on two raised sections of the 5-in-12 pitch roof of the apartment building, one section over the main entrance and the other section over the 3rd level entrance, were not utilizing a fall protection system.  This condition exposed employees to a fall of approximately 9 feet to the working level of the scaffolding system.
