29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about December 6, 2019, and at times prior, HAC Roofing LLC did not ensure that employees were protected from eye injuries due to flying particles while using pneumatic nail guns to place roofing shingles.  Abatement Note: Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about December 6, 2019, the employer did not ensure that ladders being utilized to access a residential roof were extended at least 3 feet above the landing surface. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about December 6, 2019, HAC Roofing LLC did not ensure that employees conducting re-roofing operations on a single-family residential home at 21157 Saddleback Cir., Parker, CO 80138 were protected from fall hazards greater than 9 feet.   HAC Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, Duty to have fall protection, which was contained in OSHA inspection number 1358473, citation number 1, item number 1 and was affirmed as a final order on 01/29/2019, with respect to a workplace located at 6493 Killarney Ct., Aurora, CO 80239.  HAC Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, Duty to have fall protection, which was contained in OSHA inspection number 1264540, citation number 1, item number 1 and was affirmed as a final order on 10/05/2017, with respect to a workplace located at 5025 Lowell Blvd., Denver, CO 80221.  HAC Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, Duty to have fall protection, which was contained in OSHA inspection number 1172573, citation number 1, item number 1 and was affirmed as a final order on 10/14/2016, with respect to a workplace located at 5386 Barnstormers Ave., Colorado Springs, CO 80911.  HAC Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, Duty to have fall protection, which was contained in OSHA inspection number 1172491, citation number 1, item number 1 and was affirmed as a final order on 10/14/2016, with respect to a workplace located at 1130 Modell Dr., Colorado Springs, CO 80911.
