29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Warehouse, Building 100 - On or about 10/10/17 and at times prior thereto employees performed maintenance/repairs of equipment including (but not limited to) conveyors, sorters, and electrical panels/components which are part of the sorting/shipping/receiving operations. The employer did not develop, implement, and utilize specific written procedures to protect employees from hazardous energy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Warehouse, Building 100 - On or about 10/10/17 and at times prior thereto employees performed maintenance/repairs of equipment including (but not limited to) conveyors, sorters, and electrical panels/components which are part of the sorting/shipping/receiving operations. The employer did not train authorized employees in the recognition of hazardous energy sources (including but not limited to the electrical/mechanical power systems in the conveyors), the magnitude of the energies, and the means of energy isolation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
