29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 7969 Olive Brook Dr. - employees engaged in roofing activities on a 5 in 12 sloped roof were exposed to a 10 foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 10/12/2017.   Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 997669, citation number 1, item number 1 and was affirmed as a final order on 11/25/2014, with respect to a workplace located at 206 S. Moody Ave., Tampa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1004881, citation number 1, item number 1 and was affirmed as a final order on 12/11/2014, with respect to a workplace located at 7053 Peregrina Loop, Wesley Chapel, Tampa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1055800, citation number 2, item number 1 and was affirmed as a final order on 5/21/2015, with respect to a workplace located at 4347 Vermillion Sky Drive, Wesley Chapel, FL.
