29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.  On or about June 20, 2017 at the jobsite located at Providence Reformed Presbyterian Church, 9124 Sappington Road, Saint Louis, Missouri, employee(s) for Alfredo Contreras were observed installing shingles on a steep commercial roof (8-12 pitch) without fall protection.  The fall distance was approximately 16 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about June 20, 2017 at the jobsite located Providence Reformed Presbyterian Church, 9124 Sappington Road, Saint Louis, Missouri, employee(s) for Alfredo Contreras were observed using a portable ladder with side rails extended approximately 1 foot beyond the upper landing surface or edge.  The fall distance was approximately 16 feet to the ground below.
