29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: On or about November 1, 2017 during the installation of roofing material at a job site located at 3116 Dover Center Rd, Westlake, Ohio 44145; the employer did not ensure employees working on a residential roof were protected from a fall of approximately 8.6 feet. The employer stated that the companys employees had been trained on fall protection and the hazards related to falls; but failed to ensure each exposed employee understood and or had complete comprehension of the PFAS that were being utilized.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about November 1, 2017, during the installation of roofing material at a job site located at 3116 Dover Center Rd, Westlake, Ohio 44145; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 8.6 feet by utilizing a fall protection system. The High Noon Roofing INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1075001, citation number 1, item number 1 and was affirmed as a final order on (07/29/2015), with respect to a workplace located at 55 Pearl #57, Brunswick, Ohio 44212.
