On or about August 2, 2018, employees were exposed to a fall of approximately 16 feet while installing roofing materials on a residential home located at 12915 Lake, Lakewood, Ohio 44107. The employees were working from a ladder jack scaffold and the employer did not ensure that platform was less than 14 inches from the face of the work.
