29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a) On or about December 6, 2017, the employer did not conduct annual audits of its energy control procedure to assure that proper lockout/tagout procedures and requirements were being followed. The lack of procedure audits exposed employees to injuries in that adequate machine isolation was not being followed when employees accessed hazard areas of machines while performing maintenance and servicing activities.

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) On or about December 6, 2017, the employer did not assure that each authorized employee placed a lockout device on the energy disconnects on the winder on the bubble line prior to performing maintenance operations. One authorized employee applied their locks and tags on the electrical disconnect while one other employee assisted with the task.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On December 6, 2017, the employer did not ensure that energy control procedures were developed, documented and utilized to control the electrical and pneumatic energy sources when employees were changing screen packs and performing other maintenance and servicing on the bubble line. Employees were exposed to caught in and crushing hazards while performing these tasks. Ivex Protective Packaging, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(c)(4)(i) which was contained in OSHA inspection number 850603, citation number 1, item number 19a and was affirmed as a final order on August 12, 2013 with respect to a workplace located at 456 Stolle Drive, Sidney, OH 45365.
