29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) 6 Harding Ave., Longport, NJ: On or about Dec 11, 2017 an EJS Contractors Inc. employee was working from a platform (stack of plywood) resting on the elevated forks of a JLG Extensible Boom Rough Terrain Fork Lift, over 20 feet in the air. The platform was not attached or secured in any way to the forks of the forklift.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) 6 Harding Ave., Longport, NJ: On or about Dec 11, 2017 an EJS Contractors Inc. employee was working from a platform (stack of plywood) resting on the elevated forks of a JLG Extensible Boom Rough Terrain Fork Lift. The employee was not using any form of fall protection and was thus exposed to a fall hazard of greater than 20 feet to the ground below.
