29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about December 19, 2017 at the Sidel Blow Mold machine, employees clearing a jam were exposed to an amputation hazard when the release on the locking device was depressed causing the locking device to fall.

29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure. On or about December 19, 2017 at 265 John Brooks Road, Pendergrass, Georgia, where there was a jam that needed to be cleared in the Sidel Blow Mold machine, affected employees were used that had not been instructed ion the employer's energy control procedures. Affected employee were exposed to an amputation hazard when trying to clear a jam in the Sidel Blow Mold machine.
