On or about December 27, 2017 at 101 Liberty Industrial Parkway, McDonough, Georgia, employees utilizing a ULINE H-2772 Aerial Lift Platform were exposed to a fall hazard when they were not trained in the following topics including, but not limited to: a. donning of their fall protection equipment proper and ensuring that it is attached to a suitable anchorage point.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On or about December 27, 2017 at 101 Liberty Industrial Parkway, McDonough, Georgia, an employee was exposed to a fall hazard of more than 4 feet to the lower level when lifted in a aerial platform that had not been approved by the manufacturer to be used with the Raymond 425 forklift. The platform was used while still on the pallet and was not attached to the forks.
