29 CFR 1910.1048(l)(1)(i): The employer shall institute medical surveillance programs for all employees exposed to formaldehyde at concentrations at or exceeding the action level or exceeding the STEL: On January 31, 2018, the employer did not institute a medical surveillance program for employees who were exposed to formaldehyde at concentrations at or exceeding the action level of 0.5 ppm, including but not limited to: a) 22 Machine Operator was exposed to formaldehyde at a level of 0.50 ppm as an 8-hour TWA. This exposure was determined by air sampling conducted over 468 minutes during one shift, with zero exposure being assumed for the unsampled 12 minutes of an 8-hour work shift. b) U180 #5 Machine Operator was exposed to formaldehyde at a level of 0.69 ppm as an 8-hour TWA. This exposure was determined by air sampling conducted over 476 minutes during one shift, with zero exposure being assumed for the unsampled 4 minutes of an 8-hour work shift. c) Core Wash Oven Operator was exposed to formaldehyde at a level of 0.62 ppm as an 8-hour TWA. This exposure was determined by air sampling conducted over 441 minutes during one shift, with zero exposure being assumed for the unsampled 39 minutes of an 8-hour work shift. d) U180 #2 Machine Operator was exposed to formaldehyde at a level of 0.64 ppm as an 8-hour TWA. This exposure was determined by air sampling conducted over 460 minutes during one shift, with zero exposure being assumed for the unsampled 20 minutes of an 8-hour work shift.
