29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  Employees working on the U180 machines and machine 16s perform set-up and alignment activities were not required :   (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, (2)  turn off and shut down the machines (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4)  affix lockout or tagout devices to each energy isolating device, (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.  On or about January 9, 2018 and thereafter, employees performed set-up and alignment activities on the U180 machines and machine 16s while still energized exposing employees to struck-by hazards.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    On or about January 11, 2018, a Vestil jib attachment placed on the forks of the powered industrial vehicle (such as the Yale) affected the capacity and safe operations did not have the tags or decals changed accordingly.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    On or about January 11, 2018, a Vestil jib attachment would be placed on the forks of the powered industrial vehicle (such as the Yale) did not have it marked to identify the attachment and show the approximate weight of the truck and attachment combination.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    (a)  On or about January 11, 2018, the Redford Carver 16 #1 had an unguarded pinch point where the sand magazine rotated over the core boxes exposing employees to caught between hazards.    (b)  On or about January 11, 2018, the machine 16 #2 had an unguarded pinch point where the sand magazine rotated over the core boxes exposing employees to caught between hazards.    (c)  On or about January 11, 2018, the ATD #22 had an unguarded pinch point where the die tray comes to the machine frame.  The light curtain installed in this area was blanked out exposing employees to caught between hazards.

29 CFR 1910.217(b)(7)(v)(a): Two-hand controls for single stroke shall conform to the following requirements:  Each hand control shall be protected against unintended operation and arranged by design, construction, and/or separation so that the concurrent use of both hands is required to trip the press.  On or about January 11, 2018, machine 16 #2 did not have properly functioning two-hand controls allowing the machine to activate with one control exposing employees to struck-by hazards.
