29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a) Hidden Springs Carpentry LLC on or about January 11, 2018, at the site of Lot 4, Signature Pkwy Avon Lake, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Hidden Springs Carpentry LLC on or about January 11, 2018, at the site of Lot 4, Signature Pkwy Avon Lake, OH: The employer did not ensure employees were protected from falls while installing new roofing material atop a residential type structure.  Employees operating in this condition were exposed up to a 10 ft. fall to the ground below.
