29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about December 23, 2019, employees were exposed to a struck by hazard while performing framing activities on a new residential condo development located at 5861 Father Caruso Drive, Cleveland, Ohio 44102. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about December 23, 2019, employees were exposed to a fall of approximately 21 feet while performing framing activities on a new residential condo development located at 5861 Father Caruso Drive. The employees were working on the second level and the employer did not ensure that they were using fall protection. Hidden Springs Carpentry LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1287407, citation number 1, item number 1b and was affirmed as a final order on February 12, 2018, with respect to a workplace located at Building #4, Signature Pkwy, Avon Lake, OH 44012. Hidden Springs Carpentry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1413009, citation number 1, item number 1 and was affirmed as a final order on 8/6/2019, with respect to a workplace located at 32631 Albion, Avon Lake, Ohio 44012.
