29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a)  Hidden Springs Carpentry LLC on or about June 27, 2019, at the address of 32631 Albion Drive, Avon Lake, OH: The employer did not ensure employees were protected from falls while installing new roofing material atop a residential type structure.  Employees operating in this condition were exposed up to a 18 ft. fall to the ground below.  The Hidden Springs Carpentry LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1287407, citation number 1, item number 1b and was affirmed as a final order on February 12, 2018, with respect to a workplace located at Building #4, Signature Pkwy, Avon Lake, OH.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  a)  Hidden Springs Carpentry LLC on or about June 27, 2019, at the address of 32631 Albion, Avon Lake, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces.  The Hidden Springs Carpentry LLC was previously cited for a violation of this occupational safety and health standard 1926.503(a)(1), which was contained in OSHA inspection number 1287407, citation number 1, item number 1c and was affirmed as a final order on February 12, 2018, with respect to a workplace located at Building #4, Signature Pkwy, Avon Lake, OH.
