29 CFR  1926.251(a)(1):Defective rigging equipment was not removed from service:  Hidden Springs Carpentry, LLC at the worksite located at Aqua Marine Blvd in Avon Lake, Ohio: On or about April 12, 2021, rigging equipment used to lift trusses was defective in that the hook on the forklift boom attachment was not equipped with a safety latch. Employees were exposed to crushing injuries when lifting trusses in that the load and/or wire rope sling could be dislodged from the hook.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The unsecured load fell from the forks, exposing employees who were framing a residential structure to struck-by and crushing injuries. Hidden Springs Carpentry, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13),  which was contained in OSHA inspection number 1413009, citation number 1, item number 1 and was affirmed as a final order on August 6, 2019, with respect to a workplace located at 32631 Albion Dr., Avon Lake, Ohio 44012.  Hidden Springs Carpentry, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1287407, citation number 1, item number 1b and was affirmed as a final order on February 12, 2018, with respect to a workplace located at Building #4, Signature Pkwy, Avon Lake, Ohio 44012.
