29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: On or about January 9, 2018, the host employer and a contractor did not inform each other of their respective lockout or tagout procedures before work began. A host employer maintenance technician turned off the main breaker to King National Air System, FD5 that a contract employee was repairing to de-energize it.

29 CFR 1910.176(b): Storage was not secure. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about January 11, 2018 and there prior to, the employer stacked pallets and boxes of merchandise on unstable dented storage racks in the warehouse, exposing employees to struck-by hazards from falling objects in these areas as follows: a. Aisle 1 - 1 dented structural brace support member. b. Aisle 19 - 2 dented structural brace support members. c. Aisle 21 - 2 dented structural brace support members.
