The emergency eyewash station (plumbed) was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that: 1. The eyewash unit did not provide adequate flushing fluid upon activation. 2. Weekly visual inspections of the eyewash station and shower were not conducted. The emergency eyewash/shower unit was not activated weekly to verify proper operation.

Analytical results indicated the presences of hexavalent chromium, as follows: 1)Lunch room, lunchroom table surface: 0.5503 ug(micrograms) of hexavalent chromium. 2) Drinking fountain area, nozzle area of fountain: 0.9533ug of hexavalent chromium. 3) Microwave, microwave button area: 0.2583ug of hexavalent chromium.
