The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about January 24, 2018: a) At the jobsite located at 4474 Big Rock Ridge Trail Oakwood Georgia, an employee was installing roofing material on a roof with a pitch of 10 in 12, was using a fall protection system that did not have the attachment point at the center of the wearer's back.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about January 24, 2018: a) An employee installing roofing material on a new residential structure was exposed to injuries while wearing improper style of fall protection. The employee was wearing rock climbing style fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about January 24, 2018: a) An employee for was exposed to fall hazards of approximately 24 feet when the extension ladder the employee was utilizing did not have the length needed to extend past the upper landing, and was not secured at the top.
