29 CFR 1926.300(b)(4)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about January 12, 2023, and at times prior there to, an employee was exposed to rotating paddles inside a portable concrete mixer due to not having a guarding device, causing an amputation to two fingers and crushing injuries to left hand.
