29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about February 13, 2018 the employer failed to ensure that the energy isolating devices of the Serac 1 & 2 palletizer conveyors were located and operated to isolate the energy sources. An employee was tasked with servicing the chain drive of the unit without controlling the hazardous energies prior to performing the work. Hazardous energy sources include electrical and mechanical (chain drive system).
