The ladder jack scaffold had 3 support ladders, two approx. 12 in. wide Aluminum piks that were bridged (overlapped) at the center support ladder, and an access ladder. The support ladder on the left side of the scaffold sank into the mud, the scaffold collapsed, and the employee fell to the ground. The support ladders on the scaffold did not bear on base plates and mud sills or other adequate firm foundation.   NO ABATEMENT CERTIFICATION REQUIRED
