29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: a) On or about 2/22/18 on the garage side of house under construction at 5696 Waterford Lane, Clarence Center, NY: An employee was engaged in roofing work while standing on an aluminum pik of a ladder jack scaffold. The ladder jack scaffold had 3 support ladders, two approx. 12 in. wide Aluminum piks that were bridged (overlapped) at the center support ladder, and an access ladder. The support ladder on the left side of the scaffold sank into the mud, the scaffold collapsed, and the employee fell to the ground. The support ladders on the scaffold did not bear on base plates and mud sills or other adequate firm foundation. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 2/22/18 on the garage side of house under construction at 5696 Waterford Lane, Clarence Center, NY: An employee was engaged in roofing work while standing on an aluminum pik of a ladder jack scaffold. The ladder jack scaffold collapsed and the employee fell approx. 11 ft. to the ground. No personal fall arrest system or other fall protection system was utilized. NO ABATEMENT DOCUMENTATION REQUIRED Brock Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1205811, citation number 2, item number 1 and was affirmed as a final order on 11/17/2017, with respect to a workplace located at 6 Pear Tree Lane, Lancaster, NY 14086
