29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: WORKSHOP/MILL DEPARTMENT #4: The ingoing nip point, created by aluminum wire and the take-up wheel/block of the old three stand mill was unguarded during the sample collecting preparation period. On/or about February 23, 2018, The take up wheel/block of the above equipment rotated while an employee was in the process of collecting a sample of the aluminum wire, and the employee sustained right middle and ring fingers amputations.
