29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, 131 15th Street, Pittsburgh, PA: On or about March 8, 2018, during trench and excavation activities, the employer did not ensure that adequate cave in protection was provided for an employee who entered the trench/excavation in that it was determined to be approximately 11 feet deep with vertical/near vertical dirt walls, exposing the employee to the hazard of being caught in a trench/excavation collapse.
