29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about March 24, 2020, the employer failed to ensure employees working from a trench installing sewer lines approximately seven (7) feet deep were protected by an adequate protective system. J Severino Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.652(a)(1), which was contained in OSHA inspection number 1257461 , citation number 01, item number 002 and was affirmed as a final order on October 2, 2017 , with respect to a workplace located at 1162 Clifton Blvd, Lakewood, OH 44107.
