29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform: a) Weatherboard Dip Tank: Employees were exposed to an unguarded chain and sprocket during brush adjustment maintenance while the motor was operated with the cover removed. An employee had a partial amputation of a thumb from the unguarded chain and sprocket on or about February 28, 2018. b) Weatherboard Dip Tank Tender - Employees working as tenders were exposed to ingoing nip points from an unguarded chain and sprocket, on or about March 8th, 2018. c) Weatherboard Dip Tank Operator-Employees working as operators were exposed to two (2) ingoing nip points from two unguarded chains and sprockets, on or about March 8th, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
