29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  A.  On or about March 27, 2018, Wheel Shop:  In the following instances, hardware on emergency exit doors was damaged:  1.  North side of the building, rear:  The self-retracting arm on the identified emergency exit door was damaged and detatched from the door frame.  The defective safeguard would not keep the door closed in case of emergency.    2.  GFS Paint Mixing Room:   The self-retracting arm on the exit door from the paint mixing room was damaged and detatched from the door frame.  The defective safeguard would not keep the door closed in case of emergency.

On or about March 27, 2018, Wheel Shop Production Area:  The Lehigh CNC lathe had a disabled interlock.  The enclosure doors were able to be opened during operation, allowing access to the rotating point of operation.

On or about March 27, 2018, Wheel Shop, Welding Area:  A Ryobi BG828G bench grinder was used to grind down the tungsten electrode tip used in TIG welding operations.  The equipment was not secured to the metal cart on which it was placed, to prevent it from moving during use.

On or about March 27, 2018, Wheel Shop, Welding Area: The work rest on the Ryobi BG828G bench grinder had not been adjusted to within one eighth inch of the abrasive wheel.  The gap between the work rest and the abrasive wheel measured one half inch.  Work being held against the wheel could be pulled into the gap between the work rest and the abrasive wheel, exposing employees to injury.

On or about March 27, 2018, Wheel Shop, Welding Area:  The tongue guard on the left side of the Ryobi BG828B bench grinder was missing, allowing an opening of greater than one fourth inch between the abrasive wheel and the equipment enclosure.  The opening measured one half inch.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   A.  On or about March 27, 2018, in the following instances, flexible extension cords were used in lieu of the fixed wiring of the building to power equipment:  1.  Wheel Shop, at the Steelman Bake Oven:  Five 31-inch diameter metal encased pedestal fans used to cool parts were energized by flexible extension cords which were run underneath the oven to outlets on the South wall of the building.  2.  Wheel Shop, Wheel Balancing Area near the Oxygen Tank:  A Central Machinery 31-inch diameter metal-encased pedestal fan was energized through an extension cord plugged into a wall outlet around the corner, behind a tire rack.  3.  Wheel Shop, Production Area, near Electrical Panel PP3:  A stereo and several speakers were powered through a yellow extension cord that ran across the shop floor from a wall-mounted outlet box on the wall of the Welding Area.

On or about March 27, 2018, Wheel Shop at Powder Coating Booth:   The insulation on the flexible cordset for the Nordson Encore Powder Coating Sprayer control panel had been pulled from its fitting, exposing the energized wires.  The equipment was powered through an outlet on the side of the Powder Coating Booth, along the main aisle into the shop.

29 CFR 1910.305(a)(2)(ii): Temporary wiring was not removed immediately upon completion of the project or purpose for which the wiring was installed:   A.  On or about March 27, 2018, Wheel Shop:  A yellow extension cord, plugged in to a 4-receptacle wall outlet in the Welding Area ran across the floor to an area near electrical panel PP3, where it provided power to a stereo and speaker system. The equipment was not being used at the time of the inspection and had not been unplugged after use.
