29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks; a) On or about March 29, 2018, the employer failed to ensure that effective guarding was in place to protect employees from the in-going nip point of the mobile conveyors. The hazard areas where the belt meets the pulley were not guarded on the conveyors between presses B10 and B20. Employees remove parts from the conveyors prior to placing them in the presses and at the final exit off the line. b) On or about March 29, 2018, the employer failed to ensure that effective guarding was in place to protect employees from the in-going nip point of the mobile conveyors. The hazard areas where the belt meets the pulley were not guarded on the conveyors between presses B20 and B30. Employees remove parts from the conveyors prior to placing them in the presses and at the final exit off the line. c) On or about March 29, 2018, the employer failed to ensure that effective guarding was in place to protect employees from the in-going nip point of the mobile conveyors. The hazard areas where the belt meets the pulley were not guarded on the conveyors between presses B40 and B50. Employees remove parts from the conveyors prior to placing them in the presses and at the final exit off the line. e) On or about March 29, 2018, the employer failed to ensure that effective guarding was in place to protect employees from the in-going nip point of the mobile conveyors. The hazard areas where the belt meets the pulley were not guarded on the conveyors on the exit side of B50. Employees remove parts from the conveyors prior to placing them in the presses and at the final exit off the line. The Artiflex Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1067041, citation number 1, item number 1 and was affirmed as a final order on September 15, 2015 with respect to a workplace located at 1425 East Bowman Street, Wooster, OH 44691.

29 CFR 1910.217(c)(1)(i): The employer did not provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. a) On or about March 29, 2018, the employer failed to ensure that effective point of operation guarding was in place on the operator side of press C50. Final finishing employees were working on the operator side of the press removing and checking parts without effective guarding in place. b) On or about March 29, 2018, the employer failed to ensure that effective point of operation guarding was in place on the exit side of press B40. No barrier guarding or other devices were present on the backside of the press to prevent employees from reaching into the hazard areas. c) On or about March 29, 2018, the employer failed to ensure that effective point of operation guarding was in place on the exit side of press B50. No barrier guarding or other devices were present on the backside of the press to prevent employees from reaching into the hazard areas. d) On or about March 22, 2018, the employer failed to ensure that effective point of operation devices were properly applied on press B30. Two hand control devices in use on B30 were not properly applied for all operations including the cam movement following a full cycle of the press. The employee was able to reach into the press at the time when the part is being moved by a cam. Artiflex Manufacturing was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.217(c)(1)(i), which was contained in OSHA inspection number 1092093, citation number 1, item number 2, and was affirmed as a final order on February 25, 2016 with respect to a workplace located at 1425 East Bowman St., Wooster, OH 44691.

29 CFR 1910.217(c)(3)(vii)(d): Two-hand controls used for mechanical power press(es) were not fixed in position so that only a supervisor or safety engineer was capable of relocating the controls; a) On or about March 29, 2018 the employer failed to ensure that two hand control devices utilized by employees for the operation of mechanical power press B20 were fixed into position and not moveable by operators. Press B20 contained two operators with designated two hand control pedestals. Neither pedestals were affixed. b) On or about March 29, 2018 the employer failed to ensure that two hand control devices utilized by employees for the operation of mechanical power press B30 were fixed into position and not moveable by operators. Press B30 contained four operators with designated two hand control pedestals. All four pedestals were not affixed. c) On or about March 29, 2018 the employer failed to ensure that two hand control devices utilized by employees for the operation of mechanical power press B40 were fixed into position and not moveable by operators. Press B40 contained two operators with designated two hand control pedestals. Neither pedestals were affixed. d) On or about March 29, 2018 the employer failed to ensure that two hand control devices utilized by employees for the operation of mechanical power press B50 were fixed into position and not moveable by operators. Press B50 contained four operators with designated two hand control pedestals. Three of the four pedestals were not affixed. Artiflex was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.217(c)(3)(vii)(d), which was contained in OSHA inspection number 1119285, citation number 1, item number 4 and was affirmed as a final order on February 25, 2016 with respect to a workplace located at 1425 Bowman Street, Wooster, OH 44691.

A conduit line on the control panel side of the press was missing a section or in need of repair to effectively cover the wiring.
