29 CFR 1910.134(c)(2)(i): Where the employer provided respirators at the request of employees, the employer did not determine that such respirator use will not in itself create a hazard. On or about April 10, 2018: Employees working in the production area spraying and/or sanding are exposed to respiratory dust hazards. The employees wear a 3M 6000 Series Half Facepiece, negative pressure respirators on a voluntary basis, however the employer did not ensure employees were medically evaluated.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: Applies to any extinguishers provided in the workplace. On or about April 10, 2018: Employees working in the facility located at 120 McArthur Road in Marble Hill are exposed to fire and smoke inhalation hazards where the employer provided ABC, Dry Chemical Fire extinguishers throughout the facility but did not ensure that they were serviced annually.
