West wall of production area-directly adjacent to the west exit door, on or about April 25th, 2018 and times prior thereto:  Employees were potentially exposed to a fire/explosion hazard in that, two oxygen cylinders and one propylene cylinder were stored together on the West wall of the production area.

The employer did not provide effective Hazard Communication training for those employees working with and around hazardous substances.

29 CFR 1904.41(a)(2): 29 CFR 1904.41(a)(2): Annual electronic submission of part 1904 records by establishments with 20 or more employees, but less than 250 employee. The establishment had 20 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:     a)  RGS Products, Inc., on or about April 25th, 2018 and times prior thereto:  The employer failed to electronically submit information from their OSHA 300A for calendar year 2016 by December 15th, 2017.  The establishment employed approximately 20 employees and is classified as NAICS 332323 which is included as a high-risk industry.
