29 CFR 1910.23(d)(7): Grab bars did not extend 42 inches (1.1 m) above the access level or landing platforms served by the ladder:  Salvage parts area: Access to the second level of the salvage parts area did not have grab bars that extend above the landing platform exposing employees to fall hazards.  Or, in the alternative  29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:  Salvage parts area: Access to the second level of the salvage parts area: did not have a stair rail system exposing employees to fall hazards.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxillary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substanially equivalent to the wall of the equipment:  In the boiler room: The junction box supplying power to the boiler controller 1 had an unused opening through which moisture had access to live electrical parts exposing employees to electrical and arc flash hazards.
