29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. a) Jobsite - The employer did not provide a safe means of access for all levels of the tubular supported welded frame scaffold by the use of portable ladders, hook-on ladders or other acceptable means. Employees conducting stucco operations from the scaffold used the rungs on the scaffold end frames for access. Noted on or about 5/25/18.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite - Employees conducting stucco operations from a supported tubular welded frame scaffold were exposed to fall hazards, approximately 32 feet above ground level, without the use of a fall protection system. Noted on or about 5/25/18.
