29 CFR 1926.20(b)(2): The employer did not initiate and maintain frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a)  On or about July 1, 2024,  at a jobsite located at 3721 E. Stargrass Ct., Oak Creek, WI 53154, a competent person was on site but did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about July 1, 2024, at a jobsite located at 3721 E. Stargrass Ct. Oak Creek, WI, 53154, employees working on the ground to perform cleanup and assist the roofers who were working above while re-roofing the structure, were not protected from struck by hazards from falling materials and tools by wearing a protective helmet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about July 1, 2024, at the jobsite located at 3721 E. Stargrass Ct. Oak Creek, WI. 53154, employees conducting roofing activities (shingle installation) on a 6:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the first floor roof of approximately 9 feet, 5 inches on the East side of the structure and 17 feet on the North, East, and South  gable ends of the roof ridge.  CJ Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1638735, citation number 2, item number 1, and was affirmed as a final order on 1/12/2023 , with respect to a workplace located at 964 Spring Waters Dr. Oconomowoc, WI.   CJ Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1554426, citation number 1, item number 1 and was affirmed as a final order on 12/29/2021, with respect to a workplace located at 255 Four Winds Ct. Hartland, WI 53029.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about July 1, 2024, at the jobsite located at 3721 E. Stargrass Ct. Oak Creek, WI 53154, the employer did not ensure that employees exposed to potential fall hazard had been properly trained to recognize, and minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard of up to 17 feet onto grass and concrete.  CJ Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1638735, citation number 2, item number 2, and was affirmed as a final order on 1/12/2023 , with respect to a workplace located at 964 Spring Waters Dr. OCONOMOWOC, WI.
