29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about December 13 2022, at the jobsite located at 964 Spring Waters Drive Oconomowoc WI, employees were performing roofing work on a single story residential building with pneumatic nail guns without safety glasses.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about December 13, 2022 at the construction site located at 964 Spring Waters Drive, Oconomowoc WI, employees accessing the roof from a ladder which did not extend at least three feet above the roof to conduct roofing work.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about December 13 2022, at the jobsite located at  964 Spring Waters Drive, Oconomowoc WI, employees were performing roofing work on a single story residential building with a pitch of 7:12 with a fall height of approximately 9 7/10 feet without being tied off, thereby exposing the employees to a fall hazard.  CJ Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1554426, citation number 1, item number 1 and was affirmed as a final order on 12/29/2021, with respect to a workplace located at 255 Four Winds Ct. Hartland, WI 53029.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about December 13th 2022, at the jobsite located at  964 Spring Waters Drive, Oconomowoc WI, employees were  performing roofing work on a singe story residential building with a pitch of 7:12 with a fall height of approximately 9 7/10 feet without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.  CJ Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1554426, citation number 1, item number 2 and was affirmed as a final order on 12/29/2021, with respect to a workplace located at 255 Four Winds Ct. Hartland, WI 53029.
