Employees were exposed to struck-by hazards.  Employees were exposed to eye injury while installing roofing materials using a pneumatic nail gun. Employees were not provided with eye protection.

Roofers were exposed to fall hazards of up to 15 feet from the eave to the ground below. Employees performing roofing work had not received fall protection training to learn about the hazards associated with falls and how to minimize the hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Employees were exposed to fall hazards.  Roofers were exposed to fall hazards while performing roofing work. Employees accessed the roof with the use of a Louisville aluminum ladder that extended two feet from the eave of the roof.  The fall distance was 11.6 feet.
