29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a) On or about 06/06/18 at the Furnace Department; employees were exposed to a 43.5 " fall hazard while on the work platform adjacent to the west ingot conveyor when the distance between the work platform and the conveyor created a hole 8.5 inches wide which ran the length of the work platform. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: a) On or about 06/06/18 at the Furnace Department; employees were exposed to a 43.5" fall hazard when they used the stairs (having four treads 29.5 inches wide and five risers) on the work platform adjacent to the west ingot conveyor which had a handrail installed on one side but not on the other open side. b) On or about 06/06/18 at the Furnace Department; employees were exposed to a 32" fall hazard when they used the stairs (having three treads 30 inches wide and four risers) on the work platform adjacent to the east ingot conveyor which had a handrail installed on one side but not on the other open side. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by caught between hazards, moving parts and point of operation: a) On or about 06/06/18 in the Furnace area; employees were exposed to caught between hazard in the area below the bucket of the skip hoists for ingot cleaners #1 & #2. The area below the skip hoist bucket was open and accessible during the raising and lowering of this piece of equipment. b) On or about 06/06/18 in the Maintenance shop; employees were exposed to contact with a moving blade when the exposed portion of the saw blade on the Peerless reciprocating saw was accessible. c) On or about 06/06/18 in the Furnace area; employees were exposed to point of operation hazards when the opening on the ingot shear was wide enough to allow access during cutting of furnace rods. ABATEMENT CERTIFICATION REQUIRED
