29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Edison Solutions LLC.- Production Area: The employer did not develop and implement lockout/tagout procedures for machinery that includes but is not limited to: Clam Shell Die Cutters, Bailer Model# V630CC, S/N V630CC1004869, Young Shin Machine Model # Giant 250SHII, S/N YT-37-204001, Fatoba cutting machine, etc. Employees conducting maintenance, light maintenance such as changing blades and lubrication, cleaning and unjamming machines were not provided with specific written LO/TO procedures to control all energy sources including but not limited to electric and pneumatic, occurred on or about 06/18/2018.
