29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #31, Third level: On or about 06/19/2018, and at times prior, the employer exposed employees to falls of 23-feet to the lower level, in that a fall protection system was not provided to employees engaged in framing activities using the third floor as a walking/working surface.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New residential construction located on Lot #31, First Floor: On or about 06/19/2018, and at times prior, the employer exposed employees to falls hazards of 10-feet, 9-inches to the lower concrete surface, in that the ladder provided did not extend at least 3-feet past the landing and/or was not secured to the structure to allow for safe access to lower and upper levels.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder:  (a) New residential construction located on Lot #31, Ladder located between first and second levels: On or about 06/19/2018, and at times prior, the employer exposed employees to falls of up to 10-feet, 9-inches to the lower level, in that employees were allowed to use ladders without facing the ladder and gripping the side rails making three points of contact while descending to the lower level.
