29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about July 3, 2018, at the above addressed jobsite, employees performing residential carpentry operations on a structure, twenty-one (21) feet, approximately one (1) story from an unguarded basement opening and approximately one (1) story, due to unguarded windows, were not protected from falls due to unguarded window openings, thereby exposing employees to fall hazards.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:     a.  On or about July 3, 2018, at the above addressed jobsite, the stairway to the top floor was not provided with handrails, thereby exposing employees to fall hazards.
