29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 09, 2018, employees were working on the framework of a residential home using a portable ladder to access it but the side rails of the ladder were not at least three feet above the landing. The employees were exposed to fall hazards of greater than six feet to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems: On or about July 9, 2018, employees engaged in roofing operations were not using any form of fall protections. Employees were exposed to fall hazards greater than 6 feet. DMA Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1001345, citation number 01, item number 001 and was affirmed as a final order on November 12, 2014, with respect to a workplace located at 14608 Alton Oaks, El Paso, TX 79938.
