29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:  On or about July 10, 2018 on the north side of the building, an employee was on an unprotected side, exposing him to a fall of 10'8".

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 10, 2018 on the north side of the building, an employee used a ladder which did not extend three feet above the landing to access a platform, exposing him to a fall of 10'8".
