29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    On or about February 15, 2019, at 3385 Vendome St, Port Saint Lucie, Florida, 34953, the employer failed to provide fall protection training to employees who were conducting framing work on the roof of a new residential construction, thus exposing employees to a fall hazard of 12 feet to ground level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about February 15, 2019, at a job site located at 3385 Vendome St, Port Saint Lucie, Florida, 34953, employees were exposed to a fall hazard of 12 feet to ground level, while installing trusses for the roof of a new residential home, without using any method of conventional fall protection system.      Loredo Flat Surface Concrete, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1327472, citation number 01, item number 001, and was affirmed a final order on 08/30/2018, with respect to a workplace located at 461 SW Kestor Drive, Port Saint Lucie, Florida 34953.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about February 15, 2019, at a job site located at 3385 Vendome St, Port Saint Lucie, Florida, 34953, employees were exposed to a 9 feet fall hazard, while using an extension ladder to access the concrete wall of a new residential construction to install trusses.     The Loredo flat surface concrete Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 1329481, citation number 1, item number 2 and was affirmed as a final order on 10/25/2018, with respect to a workplace located at 1060 E. Commercial Blvd, Oakland Park, 33334.
