On or about July 20, 2018, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of greater than twenty-five (25) feet, due to the safety monitor having other responsibilities, such as but not limited to, roofing operations.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: a. On or about July 20, 208, at the above addressed jobsite, employees in a hoist area were exposed to fall of greater than twenty-five (25) feet created by the presence of unprotected sides, thereby exposing employees to the hazards associated with falls. Star Roofing and Siding, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.501(b)(3), which was contained in OSHA inspection number 1101487, citation number 1, item number 1 and was affirmed as a final order on August 10, 2016, with respect to a workplace located at 819 Lake St., Oak Park, IL.
