On 7/23/2018, employees of Lancaster Roofing, Inc. were working on the roof of a residential building located at 611 Cavalry Drive, Wright City, MO. The ladder the workers were using to access the roof was not extended three feet beyond the landing or edge of the roof.

Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations. Lancaster Roofing Inc. was previously cited for a violation of this occupational Safety and Health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1276638, citation number 1, item number 1 and was affirmed as a final order on 1/16/2018, with respect to a workplace at 805 Main Street, Moscow Mills, MO.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 7/23/2018, employees of Lancaster Roofing, Inc. were working on the roof of a residential building located at 611 Cavalry Drive, Wright City, MO. Employees were engaged in roofing work approximately 10 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below. Lancaster Roofing Inc. was previously cited for a violation of this occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1276638, citation number 2, item number 1 and was affirmed as a final order on 1/16/2018, with respect to a workplace at 805 Main Street, Moscow Mills, MO.
